The moment tends to arrive without warning. A parent forgets a grandchild's name. A familiar drive home becomes disorienting. A once-organized person can no longer manage their medications or keep track of appointments. For adult children witnessing these changes, the mind moves immediately — and understandably — toward the most feared explanation: dementia.

But geriatric medicine has long recognized a troubling pattern. A substantial proportion of older adults referred to memory specialists for suspected dementia are ultimately found to have conditions that are entirely — or at least partially — reversible. The tragedy is not just in the misdiagnosis. It is in the months or years that pass before anyone looks for the treatable cause.

The Diagnostic Gap in Primary Care

Primary care physicians occupy the front line of cognitive assessment for older Americans. They are, in most cases, the first clinicians to hear a family's concerns, and the ones responsible for determining whether a referral to neurology or geriatric psychiatry is warranted. The challenge is structural: the average primary care appointment lasts roughly fifteen to eighteen minutes. A thorough cognitive and psychiatric workup requires considerably more time than that, as well as access to specialized assessment tools that are not uniformly available in outpatient settings.

The Mini-Mental State Examination and the Montreal Cognitive Assessment — the two screening instruments most commonly used in primary care — are valuable but limited. They can identify cognitive impairment; they cannot reliably identify its cause. A patient with severe untreated depression may perform nearly identically on these assessments to a patient in the early stages of Alzheimer's disease. Without additional investigation, the two presentations are clinically indistinguishable.

The consequence is a diagnostic pipeline that moves efficiently toward specialist referral but less efficiently toward the systematic exclusion of reversible causes — a step that should, in most cases, come first.

Depression: The Condition Most Likely to Be Mistaken for Dementia

Late-life depression is among the most prevalent and most underdiagnosed conditions in older Americans. The American Association for Geriatric Psychiatry estimates that depression affects up to five million adults over the age of sixty-five, yet fewer than half receive any treatment. In the context of cognitive assessment, this matters enormously.

Depressive episodes in older adults frequently present differently than they do in younger people. The prominent low mood and tearfulness that clinicians are trained to look for may be absent or minimized. Instead, the presentation is often dominated by cognitive symptoms: slowed thinking, difficulty concentrating, impaired memory, social withdrawal, and diminished initiative. This cluster of symptoms — sometimes called pseudodementia, though the term has fallen somewhat out of favor — can be severe enough to mimic moderate dementia on standardized testing.

The critical distinguishing feature is reversibility. When depression is adequately treated — through psychotherapy, pharmacotherapy, or a combination — cognitive function typically improves in parallel with mood. Patients who appeared to be on a trajectory toward memory care may return to independent functioning. This is not a rare outcome. It is, in appropriately identified cases, the expected one.

Polypharmacy: The Medication Burden Nobody Is Calculating

The average American over the age of sixty-five takes four or more prescription medications. Many take considerably more. Each additional drug introduces the possibility of side effects, drug-drug interactions, and cumulative effects on the central nervous system that no single prescriber may be tracking in full.

Anticholinergic medications — a class that includes certain antihistamines, bladder medications, antidepressants, and antipsychotics — are particularly well-documented contributors to cognitive impairment in older adults. These drugs block acetylcholine, a neurotransmitter critical to memory and attention. In younger patients, the cognitive effects are usually transient and mild. In older patients, whose brains are already managing age-related changes in neurotransmitter function, the same medications can produce confusion, memory loss, and behavioral changes that look, to an untrained eye, like dementia.

The Beers Criteria, published and regularly updated by the American Geriatrics Society, provides a comprehensive list of medications considered potentially inappropriate for older adults. Yet studies consistently find that a significant proportion of older Americans are prescribed one or more Beers Criteria medications, often by physicians who are not aware of the patient's full medication list. A comprehensive medication reconciliation — ideally conducted by a clinical pharmacist or geriatrician — can sometimes produce dramatic cognitive improvement by identifying and discontinuing the offending agents.

Untreated Sleep Apnea: The Silent Cognitive Disruptor

Obstructive sleep apnea is profoundly common among older adults, yet it remains dramatically underdiagnosed in this population. The reasons are partly perceptual: sleep apnea is culturally associated with middle-aged men with obesity, and clinicians may not think to screen an older woman of normal weight, even if she is presenting with fatigue, cognitive complaints, and mood disturbances.

The mechanism linking sleep apnea to cognitive impairment is not subtle. Repeated episodes of oxygen desaturation during sleep — occurring dozens or hundreds of times per night in severe cases — impose cumulative stress on brain tissue. Sleep fragmentation prevents the restorative deep-sleep stages during which the brain clears metabolic waste products, including amyloid-beta, the protein associated with Alzheimer's pathology. Long-term untreated sleep apnea has been associated with measurable reductions in hippocampal volume and with accelerated cognitive decline.

The clinical significance of this connection is that treating sleep apnea — typically with continuous positive airway pressure therapy — can produce meaningful cognitive improvement, sometimes within weeks of initiating treatment. For patients who have been carrying a presumptive diagnosis of early dementia, this outcome can be transformative.

A Framework for Families Navigating Uncertainty

For adult children concerned about a parent's cognitive changes, the most important immediate step is to document specific observations rather than general impressions. A clinician presented with "Dad seems confused lately" has limited information to work with. A clinician presented with a written log of specific incidents, their frequency, and their temporal relationship to medication changes, sleep disturbances, or mood shifts has substantially more.

Families should also feel empowered to ask explicitly whether reversible causes have been systematically evaluated before a dementia diagnosis is assigned or a neurology referral is placed. This means requesting a full medication review, a depression screening using a validated instrument such as the Geriatric Depression Scale, thyroid function tests, vitamin B12 levels, and a sleep assessment. These are not exotic investigations. They are the standard components of a thorough geriatric workup, and they should precede — not follow — a diagnosis of presumptive dementia.

If a primary care provider is not able to conduct this workup within the constraints of a standard appointment, a referral to a geriatrician or a geriatric psychiatrist is appropriate. These specialists are specifically trained to disentangle the overlapping presentations of aging, mood disorders, medication effects, and neurodegenerative disease.

The Stakes of Getting It Right

A dementia diagnosis carries consequences that extend far beyond the clinical record. It shapes how families relate to their loved one, how legal and financial decisions are made, and how the individual understands their own future. When that diagnosis is premature or incorrect, those consequences compound into harm.

The goal of thorough, systematic evaluation is not to minimize the reality of dementia or to offer false reassurance. Alzheimer's disease and related dementias are genuine, devastating, and increasingly prevalent. But they deserve to be diagnosed accurately — which means ruling out what can be treated before concluding that what remains cannot be.
