Custom-Compounded Drugs Are Everywhere Right Now — What Patients Must Understand Before Saying Yes

If you have visited a specialty clinic, browsed a telehealth platform, or simply scrolled through social media in the past two years, you have almost certainly encountered compounded medications — even if you did not recognize them by that name. Custom-formulated semaglutide preparations. Personalized hormone creams. Bespoke thyroid combinations. The compounding pharmacy industry, once a niche segment of American healthcare, has become a multibillion-dollar market with a presence that is difficult to ignore.
The growth is not without justification. For certain patients, compounded medications represent a genuinely valuable solution to real clinical problems. But the rapid commercialization of this space has also introduced a troubling volume of misinformation, aggressive marketing, and safety concerns that patients deserve to understand before agreeing to a prescription.
What Compounding Actually Means
At its most fundamental level, pharmaceutical compounding is the preparation of a customized medication by a licensed pharmacist, formulated to meet the specific needs of an individual patient. This practice predates modern mass manufacturing — apothecaries have been mixing custom preparations for centuries.
In contemporary American healthcare, compounding serves several legitimate purposes. A patient who is allergic to a dye or preservative present in a commercially available drug may need a version without that ingredient. A child who cannot swallow a pill may require a liquid formulation of a medication that is only commercially produced in tablet form. A patient requiring a dose that falls outside the standard range available commercially may benefit from a preparation calibrated precisely to their prescription.
These are real, clinically defensible use cases. The problem arises when the compounding model is stretched well beyond these boundaries — and the current market suggests it has been stretched considerably.
The Regulatory Divide: Compounded vs. FDA-Approved
This distinction is foundational and frequently misunderstood by patients. Medications approved by the U.S. Food and Drug Administration (FDA) have undergone rigorous clinical trials demonstrating safety, efficacy, and manufacturing consistency. The FDA does not approve compounded medications. Individual preparations are not reviewed for effectiveness before they reach patients, and the standards governing compounding pharmacies — while regulated by state boards of pharmacy and, for larger operations, the FDA under the Drug Quality and Security Act — are materially less stringent than those applied to pharmaceutical manufacturers.
This does not mean compounded medications are inherently unsafe. Many are prepared with exceptional care by highly qualified pharmacists operating under rigorous protocols. However, it does mean that the patient accepting a compounded prescription is operating with less regulatory assurance than they would with a standard drug.
"The absence of FDA approval is not a trivial distinction," explains Dr. Rachel Moreno, a clinical pharmacologist based in Boston. "It means there has been no independent, large-scale verification that the formulation does what it claims to do at the dose being prescribed. That's information patients often don't have when they're handed a prescription."
When Compounding Is the Right Answer
For all the warranted scrutiny, there are circumstances in which compounded medications represent the best available option for a patient.
Drug shortages are one such circumstance. When a commercially manufactured medication becomes unavailable — a situation that has affected everything from chemotherapy agents to pediatric antibiotics in recent years — compounding pharmacies can fill a critical gap. The FDA maintains a drug shortage database and has, at various points, authorized compounding as a temporary measure to address supply disruptions.
Pediatric formulations represent another strong use case. The pharmaceutical industry historically has not prioritized developing child-appropriate dosage forms for all medications, leaving gaps that compounding pharmacies are well positioned to address.
Allergy accommodations, veterinary applications, and certain dermatological preparations also fall within the category of compounding's legitimate clinical utility.
Where the Market Has Outrun the Medicine
The more contested territory involves compounded preparations that are not filling a clinical gap so much as capitalizing on a commercial opportunity.
The GLP-1 agonist phenomenon offers a vivid illustration. As demand for semaglutide — the active ingredient in Ozempic and Wegovy — outstripped supply beginning in 2022, compounding pharmacies began producing their own versions of the drug. Some of these preparations included additional ingredients, such as B vitamins or other compounds, that are not present in the FDA-approved formulations and have not been evaluated for safety or efficacy in combination with semaglutide.
The FDA has issued multiple warnings about compounded semaglutide, noting reports of adverse events and expressing concern about dosing accuracy. Yet direct-to-consumer telehealth platforms continued marketing these preparations aggressively, often framing them as equivalent or even superior to brand-name alternatives.
Patient accounts from across the country reflect the confusion this environment creates. "I was told it was the same thing, just more affordable," recounted one patient from a mid-sized Midwestern city who spoke to Enndo Health on the condition of anonymity. "Nobody mentioned it wasn't FDA-approved or that the dosing might be different. I had to figure that out on my own."
The Bioidentical Hormone Conversation
Another area where compounding has become deeply commercialized is hormone therapy. "Bioidentical" hormone preparations — typically estrogen and progesterone compounds marketed as natural alternatives to conventional hormone replacement therapy — are widely promoted through direct-to-consumer advertising and specialty clinics.
While some FDA-approved bioidentical hormones exist, many compounded bioidentical preparations have not been evaluated in clinical trials. The Endocrine Society and the North American Menopause Society have both cautioned against the use of compounded bioidentical hormones when FDA-approved alternatives are available, citing insufficient evidence of safety and inconsistent quality control.
This does not mean every patient using a compounded hormone preparation is being harmed. It does mean that the marketing frequently overstates what the science currently supports.
Questions Every Patient Should Ask
If your healthcare provider recommends a compounded medication, the following questions can help you make a more informed decision.
Why is a compounded preparation being recommended over an FDA-approved alternative? A credible clinical rationale should be readily available. If the primary answer relates to cost, convenience, or customization that does not address a specific clinical need, that warrants further exploration.
Where will the medication be compounded? Accreditation by organizations such as the Pharmacy Compounding Accreditation Board (PCAB) provides some assurance of quality standards, though it is not a guarantee.
Has this specific formulation been used in clinical studies? Not all compounded preparations have supporting research. Understanding the evidence base — or lack thereof — is important context.
What are the known risks associated with this preparation? Just as you would ask about side effects for any prescription, the same question applies to compounded medications.
Is this addressing a drug shortage or a genuine clinical need? If the commercially manufactured version of a drug is available, the rationale for choosing a compounded alternative should be specific and well-articulated.
A Framework for Informed Decision-Making
Compounded medications occupy a legitimate and, in specific circumstances, indispensable role in American healthcare. The challenge for patients is navigating a marketplace that has expanded well beyond those specific circumstances, driven in part by financial incentives that do not always align with clinical best practice.
The most effective protection is an informed, engaged conversation with a healthcare provider who is not financially tied to the compounding pharmacy they are recommending. Transparency about the regulatory status of a proposed treatment, the evidence supporting it, and the alternatives available should be the minimum standard for any prescription — compounded or otherwise.
Healthcare decisions made with complete information are almost always better than those made in a vacuum of marketing language. In the compounding space, that principle has never mattered more.
